How do I update my financial details?
If you are paying your subscription fees by direct debit from your bank or credit card you need to sign and complete a new Debit Authority Form to change the account or credit card the payments are coming from.
Download the form. Print the form off, complete it and sign it.
Once completed, please fax the signed form back to us on (03) 9662 4591. Or e-mail us the scanned form to enquiry@cpsuvic.org
However, if your credit cards details remains the same but need to upate the expiry date only you don't need to fill in the form, you can simply advise us the change by calling us on (03) 9639 1822.
